Provisions about specialization (internship)

In addition to heads of health care departments at regional, Kyiv and Sevastopol city state administrations, chief state sanitary doctors of regions, cities Kyiv and Sevastopol, heads of industrial unions, enterprises, joint stock companies “Pharmacy” (hereinafter referred to as IU “Pharmacy”), rectors of higher medical (pharmaceutical) schools of III-IV accreditation levels and establishments of postgraduate education, heads of medical organizations and establishments, subordinate to the Ministry of Health Care of Ukraine, that perform the training of doctors (pharmacists) at the internship, should accept these Provisions for control and fulfillment. The Provisions about primary specialization (internship) of graduates from medical (pharmaceutical) higher schools of Ukraine, approved by order from Ministry of Health Care of Ukraine No 104 (z0193-94), dd. 20.06.1994, should be considered to be invalid. 4. The control for fulfillment of this order is entrusted to the heads of Chief administration of educational establishments (Voronenko Yu.V.) and state service and medical staff (Melnyk Yu.V.).

Provisions about specialization (internship) of graduates from higher medical and pharmaceutical schools of III-IV accreditation levels of medical faculties at universities

(The words “medical faculties at universities” are added to the title and after text after numbers and words “III-IV accreditation levels” according to Order by MHC No 47 (z0138-01), dd. 07.02.2001).

I. General provisions

1.1. The specialization (internship) is a compulsory form for postgraduate training of graduates from all faculties in medical and pharmaceutical higher schools of III-IV accreditation levels in medical faculties at universities regardless of subordination and pattern of ownership, after graduation from which the qualification of doctor (pharmacist) – specialist of certain occupation is awarded. 1.2. The main task of internship is to improve the level of practical training at graduates from higher medical (pharmaceutical) schools of III-IV accreditation levels in medical faculties at universities, their occupational readiness for independent medical (pharmaceutical) activity. 1.3. The graduates, who are enrolled to the postgraduate study at higher schools and scientific research institutes on specialties that are not referred to direction “Medicine” (“Pharmacy”), or sent to the scientific research work to these establishments or to the work to other establishments (institutions) to the post that are not referred to the list of medical (pharmaceutical) posts, are released from internship. 1.4. The internship is held in mural/extra-mural forms of study at departments of higher medical (pharmaceutical) schools of III-IV accreditation levels in medical faculties at universities and establishments of medical (pharmaceutical) postgraduate education (hereinafter referred to as higher schools) and probation in basic establishments and institutions of health care. 1.5. While studying at the internship the doctors (pharmacists) may study at the Master’s course on the same specialty as the specialty of internship at the same time. The study at Master’s course is held at the same department of higher school, at which a doctor (pharmacist)-intern passes through the full time study at internship. The entry to Master’s course and study at it is governed by Temporary provisions about procedure for training of Masters of Medicine (Pharmacy) at higher medical (pharmaceutical) school of IV accreditation level, which are approved by Ministry of Health Care of Ukraine with agreement from Ministry of Education of Ukraine.

II. Enrollment to internship

2.1. The graduates from medical and pharmaceutical higher schools of III-IV accreditation levels in medical faculties at universities are enrolled to internship after passing through state exams and awarding the qualification of doctor (pharmacist) and receiving the diploma on certain medical (pharmaceutical) specialty. 2.2. Young specialists are enrolled to internship by the order from Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv, Sevastopol city state administration, regional IU “Pharmacy” and of cities Kyiv and Sevastopol, regional sanitary epidemiological station and of cities Kyiv and Sevastopol. The order about enrollment to internship mentions the specialty of internship, higher school, at which the full time part of study will be performed and basic establishment or institution (enterprise), where the probation will be held. 2.3. The ground for enrollment to internship is the diploma of doctor (pharmacist) in certain medical (pharmaceutical) specialty, certificate of job placement due to personal allocation or statement about right of self-employment. 2.4. The graduates from medical and pharmaceutical higher schools of III-IV accreditation levels in medical faculties at universities, who are sent to work to the systems of other ministries and agencies, are enrolled to internship by orders from these ministries and agencies after agreement of higher schools and conditions for full time part of study and probation with the Chief administration of educational establishments at Ministry of Health Care of Ukraine. 2.5. The graduates from medical and pharmaceutical higher schools, who due to valid reasons cannot timely, 1, September, current year, start their study at internship, shall inform the Chief administration of state service and medical staff at Ministry of Health Care of Ukraine, Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv, Sevastopol city state administration, IU “Pharmacy” and of cities Kyiv and Sevastopol, regional sanitary epidemiological station and of cities Kyiv and Sevastopol, accordingly, about it until 15, September. 2.6. The graduates from higher medical (pharmaceutical) schools, who came to study at internship behind time submit the documents that explain the reason of delay to Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv, Sevastopol city state administration, IU “Pharmacy” and of cities Kyiv and Sevastopol, sanitary epidemiological station and of cities Kyiv and Sevastopol. The decision about admission of interns to study is taken due to results of their consideration. If a graduate did not start studying at the internship until 1, September, without valid reasons, he/she is not admitted to internship. In this case as advised by Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv, Sevastopol city state administration, IU “Pharmacy” and of cities Kyiv and Sevastopol, sanitary epidemiological station and of cities Kyiv and Sevastopol he/she may be enrolled to internship since the start of next academic year with permission by Chief administration of state service and medical staff at Ministry of Health Care of Ukraine. In case of delay in start of study at internship due to valid reasons (disease, untimely conclusion of labor agreement by graduate, who is entitled for self-employment) the period of study may be prolonged to him/her for the period of delay. In this case he/she is sent to the Chief administration of state service and medical staff at Ministry of Health Care of Ukraine for solution of this issue.

III. Organization of academic process

3.1. The training of doctors and pharmacists at internship is conducted pursuant to individual curricula, developed on the basis of typical curricula and programs of postgraduate training, in accordance with Qualifying descriptions of specialists on medical specialties and pharmacists.

3.2. The classes at internship start since 1, August. 3.2.1. The duration of study at internship is 1-2 years. Graduates from medical faculties (specialty “medical care”) and treatment faculties study at internship during 2 years in specialty “general practice – family medicine”. Graduates from medical faculties (specialty “medical care”) and treatment faculties study at internship during 1.5 years in specialties of surgical profile, “obstetrics and gynecology”, “emergency medicine”. Graduates from medical faculties (specialty “pediatrics) and pediatric faculties study at internship during 1.5 years in specialties “children’s surgery”, “children’s anesthesiology”. Graduates from dental faculties study at internship during 1.5 years in specialties “dentistry”, “orthopedic dentistry”, “orthodontics”. The period of study at internship in other specialties is 1 year. 3.2.2. The duration of full time part of study at internship in departments of higher schools and probation at basic health care establishments and institutions is governed by decree from Minister of Health Care of Ukraine, dd. 6.03.96 No 50 (z0134-96) “On approval of list of specialties and periods of study for graduates from medical and pharmaceutical higher schools at internship”, curricula and internship programs. The duration of full time part of internship for doctors (pharmacists)-interns, who study at Master’s course at the same time, is prolonged for 3 months due to reduction in duration of probation at basic health care establishments and institutions. 3.2.3. The organization, general management of academic process at internship and control for its performance is entrusted to Chief administration of educational establishments at Ministry of Health Care of Ukraine, higher schools and Ministry of Health Care of Crimean Autonomous Republic, health care departments at regional, Kyiv, Sevastopol city state administrations, chief state sanitary doctors in regions, city Kyiv and Sevastopol, regional IU “Pharmacy and of city Kyiv and Sevastopol.

3.3. Ministry of Health Care of Crimean Autonomous Republic, health care departments at regional, Kyiv, Sevastopol city state administrations, chief state sanitary doctors in regions, city Kyiv and Sevastopol, regional IU “Pharmacy and of city Kyiv and Sevastopol, regional sanitary epidemiological station and of city Kyiv and Sevastopol: 3.3.1. Defines the perspective need of doctors-specialists (pharmacists) at lower organizations and every year sends the requests for training of doctors (pharmacists) with different specialties until 1, May, to the Chief administration of state service and medical staff at Ministry of Health Care of Ukraine. 3.3.2. Together with the dean’s office (division) of internship at higher schools it performs the selection and approves the list of training bases and personal composition of heads for doctors (pharmacists)-interns at training bases, as well as defines the allowable amount of doctors (pharmacists)-interns, which may study at a certain training base in a certain specialty. 3.3.3. Allocates the graduates from higher medical (pharmaceutical) schools to the lower organizations (institutions) of health care, establishments (enterprises) of IU “Pharmacy” and according to the order sends them to be enrolled to the posts of doctors (pharmacists)-interns at training bases. It is not allowed to send the bigger amount of doctors (pharmacists)-interns to the training base than it is anticipated by joint order from Ministry of Health Care of Crimean Autonomous Republic, health care departments at regional, Kyiv, Sevastopol city state administrations, chief state sanitary doctors in regions, city Kyiv and Sevastopol, regional IU “Pharmacy and of city Kyiv and Sevastopol, regional sanitary epidemiological station and of city Kyiv and Sevastopol and higher school. 3.3.4. After allocation of graduates from higher medical (pharmaceutical) schools to training bases and their enrollment to the posts of doctors (pharmacists)-interns by health care establishments (institutions) it informs the corresponding assigned higher schools about amount of doctors (pharmacists) mentioning the specialty that will be taught in current academic year at internship. 3.3.5. It plans the actions to improve the level of trainings for specialists at internship for academic year and control their fulfillment. 3.3.6. It duplicates and sends the curricula and programs, educational and methodological materials and the necessary documentation (forms of individual curricula, dairies, etc.) to training bases for doctors (pharmacists)-interns and their supervisors at training bases, which were developed by Ministry of Health Care of Ukraine or higher school. 3.3.7. Together with the higher school it performs the educational methodological and scientific practical conferences for doctors (pharmacists)-interns and their supervisors at training bases. 3.3.8. It takes part in performance of final control and attestation of doctors (pharmacists)-interns as to definition of knowledge and practical skills with awarding the title of doctor (pharmacist)-specialist. 3.3.9. Every year at the meetings of collegium it listens to the reports by chief specialists, heads and chief doctors of health care establishments and institutions, which are training bases, heads of internship about their work on improvement of practical training for graduates from higher medical (pharmaceutical) schools and improvement in efficiency of internship. 3.3.10. Every year it prepares and submits the report about training of specialists at internship to the Chief health administration at Ministry of Health Care of Ukraine during one month after attestation of doctors (pharmacists)-interns. 3.3.11. It appoints a person, responsible for internship in region, city Kyiv and Sevastopol, in order to perform organizational actions and to manage and to control the training at internship.

3.4. The chief specialist from Ministry of Health Care of Crimean Autonomous Republic, health care departments at regional, Kyiv, Sevastopol city state administrations, chief doctor of regional, Kyiv and Sevastopol sanitary epidemiological station, head of regional IU “Pharmacy” and of city Kyiv and Sevastopol: 3.4.1. Takes part in development and implementation of organizational events on training of doctors (pharmacists)-interns with the corresponding specialty at training bases. 3.4.2. Takes part in selection of training bases, supervisors of doctors (pharmacists)-interns at training bases, allocation of doctors (pharmacists) to training bases, defines the procedure for probation at specialized establishments and institutions (enterprises). 3.4.3. Together with the specialized departments of higher school he/she performs the regular checks of training bases, events to remove the defects, found in training of doctors (pharmacists)-interns, makes the proposals to improve the internship. 3.4.4. He/she takes part in performance of half-yearly final control and attestation of doctors (pharmacists)-interns. 3.4.5. He/she takes part in planning to improve the qualification and professional knowledge and skills of supervisors for doctors (pharmacists)-interns at training bases.

3.5. Training bases of doctors (pharmacists)-interns: 3.5.1. The training bases of doctors (pharmacists)-interns are the basic health care establishments and institutions, which are used for primary specialization of graduates from higher medical (pharmaceutical) schools. They can be regional, urban and central district hospitals, city and regional hospitals for children, maternity houses, health centers, hospitals, subordinate to other organizations, clinics of scientific research institutes, diagnostic centers, bureau of forensic medicine, establishments and organizations of sanitary epidemiological service, pharmacy depots, etc. that have the structural divisions, the training in which is anticipated by curriculum and internship program on corresponding specialty. 3.5.2. The training bases of doctors (pharmacists)-interns shall correspond to the following requirements: they shall have the treatment and diagnostic divisions, laboratory and diagnostic offices, other industrial divisions, where the study of interns is anticipated by curriculum and internship program on certain specialty; they shall be furnished with medical (pharmaceutical) equipment, apparatuses and tools, necessary for diagnostic, treatment, sanitary preventive (antiepidemic), pharmaceutical work at modern level and according to the tasks of that establishment or organization (enterprise); they shall have the opportunity to provide doctors (pharmacists)-interns with working places and volume of work according to requirements of curriculum and internship program on corresponding specialty; they shall have the classrooms for doctors (pharmacists)-interns, equipped with necessary educational textbooks, tables, models, stands, modern educational technologies, etc.; they shall have the possibility to provide doctors (pharmacists)-interns with hostel.

3.6. The chief doctor of basic health care establishment (institution), head of establishment (enterprise) of IU “Pharmacy”: 3.6.1. Takes actions to improve the level of diagnostic, treatment (sanitary preventive, antiepidemic, pharmaceutical) work at basic health care establishment or institution, and their provision with all necessary according to requirements to the training base of doctors (pharmacists)-interns. 3.6.2. Pursuant to the order from Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol about sending of graduates from higher medical (pharmaceutical) schools to study at internship at training base he/she issues the order about their enrollment to the posts of doctors (pharmacists)-interns in certain specialty. 3.6.3. Provides with acquaintance of doctors (pharmacists)-interns with directions in activity of basic establishment (institution, enterprise), district public service, internal code of labor conduct, industrial safety rules and standards, rights and duties of doctors (pharmacists)-interns, etc. 3.6.4. Together with the head at specialized department he/she approves the individual curricula for training of doctors (pharmacists)-interns at internship, which are developed pursuant to typical curricula and programs. 3.6.5. Provides with involvement of doctors (pharmacists)-interns into participation in cultural mass, sport and sanitary educational work. 3.6.6. Every year at the meeting of medical council (of labor collective) he/she performs the discussion about training conditions of doctors (pharmacists)-interns and fulfillment of tasks by Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol and recommendation by higher school about improvement in work of training base for doctors (pharmacists)-interns. 3.6.7. According to the order from Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol and job placement by higher school he/she sends doctors (pharmacists)-interns to the proper higher school for full time part of study. 3.6.8. He/she may appoint the person, responsible for general management by doctors (pharmacists)-interns at training base in order to plan and perform organizational and methodological actions, for current control for their fulfillment.

3.7. A person, responsible for internship at training base. 3.7.1. Organizes the training of doctors (pharmacists)-interns according to the typical curricula and programs, working in close cooperation with the dean’s office (division) of internship and specialized departments at higher school and chief specialists from Ministry of Health Care of Crimean Autonomous Republic, health care departments at regional, Kyiv, Sevastopol city state administrations, chief doctor of regional, Kyiv and Sevastopol sanitary epidemiological station, head of regional IU “Pharmacy” and of city Kyiv and Sevastopol. 3.7.2. He/she composes the work schedule for doctors (pharmacists)-interns at specialized divisions and other structural subdivisions at training base according to individual training plans for doctors (pharmacists)-interns. 3.7.3. Together with the teachers of specialized departments at higher school he/she acquaints the supervisors of doctors (pharmacists)-interns at training bases with the requirements of curricula and programs, provides them with necessary programs and methodological materials on internship. 3.7.4. He/she involves doctors (pharmacists)-interns into active participation in clinical, pathological anatomical and scientific practical conferences, which are held at basic health care establishment (institution), enterprise of IU “Pharmacy”. 3.7.5. He/she systematically performs the control for the work of supervisors for doctors (pharmacists)-interns at training bases, takes part in taking tests, which are anticipated by program, and takes actions to improve the occupational training at internship. 3.7.6. He/she prepares the materials to the council of health care establishment (institution), enterprise of IU “Pharmacy” on issues about training of doctors (pharmacists)-interns.

3.8. The supervisor of doctors (pharmacists)-interns at training base: 3.8.1. Is appointed by the order from Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol due to agreement with the dean’s office (division) of internship at higher school from a number of specialists of the highest or first qualification category (mainly head at specialized division) on the basis of not more than 7 doctors (pharmacists)-interns per one supervisor. 3.8.2. At the start of training he/she defines the level of knowledge and skills at doctors (pharmacists)-interns, these results are taken into consideration while composing individual curricula for their training. 3.8.3. Together with the teacher of specialized department at higher school pursuant to typical curriculum and program he/she composes the individual curriculum for training of doctor (pharmacist)-intern and submits it to head of training base and head at specialized department for approval. 3.8.4. He/she provides with the conditions for fulfillment of individual curricula by doctors (pharmacists)-interns. 3.8.5. He/she involves doctors (pharmacists)-interns into systematic active participation in diagnostic, treatment (sanitary preventive, antiepidemic, pharmaceutical) work, fulfillment of all kinds of activity, anticipated by requirements to job description and individual curricula for doctors(pharmacists)-interns. 3.8.6. He/she establishes the proper conditions for acquisition of necessary skills, occupational knowledge and practical skills by doctors (pharmacists)-interns, and performs the control for fulfillment of individual curricula for doctors (pharmacists)-interns. 3.8.7. He/she performs the regular and issue-related clinical rounds with doctors (pharmacists)-interns, analysis of medical histories and other accounting and reporting documents, interlocutions, seminars, practical classes for acquiring of practical skills, anticipated by individual curriculum. 3.8.8. He/she controls and is responsible for independent medical (pharmaceutical) activity of doctors (pharmacists)-interns and quality in execution of service documentation by them, takes part in performance of final control. 3.8.9. He/she performs the individual educational work with doctors (pharmacists)-interns, inoculates the respect for colleagues to them, principles of medical ethics and deontology. 3.8.10. He/she systematically improves his/her professional level, pedagogical mastery, keeps the constant contact with teachers and professors of specialized department, takes part in work of educational and methodological conferences for heads of internship, which are held by higher school.

3.9. Higher school: 3.9.1. Provides Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol, training bases with the methodological and consulting assistance on issues about training of specialists at internship. 3.9.2. It acquaints its graduates with the Provisions about specialization (internship) of higher medical and pharmaceutical schools of III-IV accreditation levels in medical faculties of universities, with rights and duties of doctors (pharmacists)-interns, controls the arrival of graduates to Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol. 3.9.3. Every year until 1, June, he/she sends the calendar plans for study and job placements, according to which doctors (pharmacists)-interns are sent to full time part of study at higher school to territorial health care management bodies, assigned to higher school: Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol. 3.9.4. Pursuant to the orders about enrollment of graduates from higher medical and pharmaceutical schools of III-IV accreditation levels in medical faculties of universities to internship and sending doctors (pharmacists)-interns from training base to full time part of study of territorial health care management bodies: Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol, which are assigned to higher school, it issues the order about their enrollment to full time part of study at internship in specialized departments. 3.9.5. It defines the specialized departments and managers of groups, who are responsible for training at internship in a certain specialty and rendering the methodological and consulting assistance to training bases. 3.9.6. It provides with study at departments according to curriculum and internship programs. It develops the educational and methodological recommendations on issues about internship and sends them for implementation at departments and training bases. 3.9.7. It organizes the scientific practical and educational methodological conferences for doctors (pharmacists)-interns and their supervisors. 3.9.8. It issues the order about composition of attestation commission on definition of knowledge and practical skills with awarding the title of doctor (pharmacist)-specialist and sends the proposals about appointment of chairman in commission to the Chief administration of higher schools at Ministry of Health Care of Ukraine. 3.9.9. Every year at the meetings of scientific council together with territorial health care management bodies, which are assigned by Ministry of Health Care of Ukraine: Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol, it discusses the issues about training of specialists at internship. 3.9.10. Every year during a month after performance of attestation of doctors (pharmacists)-interns, it submits the report about training of specialists at internship and reports by chairmen in commissions on attestation of interns on specialties, which are analyzed by specialized administrations. 3.9.11. The dean’s office (division) of internship at higher school manages and controls the training of specialists at internship. In cities, where there are no higher schools, if necessary, taking into consideration the available appropriate material and technical and staff provision, a higher school may establish the affiliates of higher school, affiliates of departments or independent departments for study of doctors (pharmacists)-interns on the base of regional (city, central district) health care establishments.

3.10. Dean’s office (division) of internship at higher school: 3.10.1. Takes part in work of commission for personal allocation of graduates from higher medical (pharmaceutical) schools. 3.10.2. It coordinates the activity of specialized departments in organization and performance of internship. It composes the time schedule for doctors (pharmacists)-interns at adjacent departments. 3.10.3. It provides with the interaction in work of specialized departments and Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol and training bases, analyzes, generalizes the materials and introduces the proposals to improve the organization and performance of specialists training at internship. 3.10.4. It performs the control for timely departure to training bases and qualitative performance of methodological and consulting assistance by teachers of specialized departments. 3.10.5. Together with territorial health care management bodies, which are assigned by Ministry of Health Care of Ukraine to higher school: Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol, it provides the departments, training bases and interns with curricula and programs, forms of documentation, record books, certificates and other reporting and accounting documentation. 3.10.6. It runs the accounting and reporting on internship, controls running the necessary documentation at departments, training bases and doctors (pharmacists)-interns. 3.10.7. In order to coordinate the work of specialized departments at higher schools, health care authorities and establishments on training of specialists at internship, to solve the issues about planning, organization, teaching methods at departments of higher schools and training bases the rector of higher school and minister of health care of Crimean Autonomous Republic, head if health care department at regional, Kyiv, Sevastopol city state administration, chief state sanitary doctor in region, city Kyiv and Sevastopol, head of regional IU “Pharmacy” and of city Kyiv and Sevastopol establishes the permanent commission on internship, which includes teachers of higher school, chief specialists and persons, responsible for internship at Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol. The chairman of this commission is the dean (head at division) of internship.

3.11. Specialized departments at higher school: 3.11.1. Provide with study of interns at departments according to Provisions about organization of academic process at higher schools, approved by order from Ministry of Education of Ukraine, dd. 2.06.93 No 161 (z0173-93), and these Provisions. 3.11.2. Involve doctors (pharmacists)-interns into different kinds of work at clinic (laboratory) of department: rounds by head at department, professors and assistant professors, consultations, clinical and issue-based analysis of patients, clinical, clinical anatomical-pathological and scientific practical conferences, sanitary epidemiological councils, industrial processes, etc. 3.11.3. Perform the regular control for work of training bases for doctors (pharmacists)-interns, which are assigned to this department, systematically inform the heads of training bases, chief specialists from Ministry of Health Care of Crimean Autonomous Republic, health care departments at regional, Kyiv, Sevastopol city state administrations, chief doctor of regional, Kyiv and Sevastopol sanitary epidemiological station, head of regional IU “Pharmacy” and of city Kyiv and Sevastopol, dean’s offices (divisions) of internship about organization and quality of practical training of doctors (pharmacists)-interns to take the appropriate actions, directed to improve their training. 3.11.4. Develop the textbooks for doctors (pharmacists)-interns and supervisors of interns at training bases. 3.11.5. Perform the control for level in training of doctors (pharmacists)-interns at the end of educational cycle at department and take part in performance of attestation for doctors (pharmacists)-interns. 3.11.6. During 3 days after completion of study and attestation of doctors (pharmacists)-interns they submit the report about academic work with doctors (pharmacists)-interns, methodological and consulting assistance at training bases, analysis of results from attestation of doctors (pharmacists)-interns to the dean’s office (division) of internship.

3.12. Scientific research work of doctors (pharmacists)-interns: 3.12.1. The scientific research work of doctors (pharmacists)-interns (hereinafter referred to as SRWI) provides with acquisition of skills for independent work with scientific literature, forms the ability of analysis and generalization of material, development of clinical (hygienic, pharmaceutical) thinking, free presentation of thoughts, critical understanding the data of literature. 3.12.2. SRWI may use the analysis of data, received at additional methods of patients’ examinations, reports and scientific practical analysis on activity of polyclinics and in-patient hospitals, specialized services, comparative analysis on performances of health care establishments (institutions) with the same performances for past years, with average values in city, region, and country. SRWI includes the search for literature and patents, mastering the special methods. 3.12.3. The topics of researches are defined in plan for scientific research work of doctor (pharmacist)-intern, composed by performer with the teacher of department taking into consideration the real possibilities for fulfillment while studying at department of higher school and at training base. 3.12.4. The doctors (pharmacists)-interns report about results from scientific research work at scientific practical conferences and they are taken into consideration at attestation of doctors (pharmacists)-interns. Due to results from scientific research work of doctors (pharmacists)-interns the scientific council of higher school may give the recommendation to the doctor (pharmacist)-intern to enter to clinical residency after graduation from internship without necessary expirience of practical work. 3.12.5. Doctors (pharmacists)-interns, who at the same time studied at Master’s course and completely performed the curriculum and prgram for master and intern, defended Master’s thesis at scientific council of higher school (faculty), may be recommended by scientific council of higher school to enter to postgraduate study or to take part in competition for the post of assistant at department (a researcher of scientific research institution) to the specialty that corresponds to the profile of Master’s course without necessary two year’s expirience in practical work.

IV. Rights and duties of doctors (pharmacists)-interns.

4.1. Doctors (pharmacists)-interns in administrative relation report to the Ministry of Health Care of Crimean Autonomous Republic, health care departments at regional, Kyiv, Sevastopol city state administrations, chief doctor of regional, Kyiv and Sevastopol sanitary epidemiological station, head of regional IU “Pharmacy” and of city Kyiv and Sevastopol. 4.2. While training at basic health care establishment (institution) a doctor (pharmacist)-intern works under supervision of doctor (pharmacist), appointed by its direct head, and during full time part of study at department of higher school – under supervision of the teacher, assigned to him/her. 4.3. During full time part of study at departments the doctors (pharmacists)-interns are covered by internal code of labor conduct, established for employees of this health care establishment (institution). 4.4. Doctors (pharmacists)-interns are entitled to use laboratories, offices, lecture halls, reading rooms, libraries and other educational and auxiliary subdivisions of higher school, to take part in operational meetings of departments in discussion of issues about improvement of academic process and other issues, related to study and living conditions of doctors (pharmacists)-interns, to be provided with accommodation at hostel of higher school while studying at department (with payment according to existing rates) and of health care establishment (institution) during training. 4.5. Doctors (pharmacists)-interns shall acquire, deepen and improve the professional knowledge and skills, perform the curriculum and internship program in full and timely, to follow to internal code of labor conduct at health care establishments and institutions, dormitory rules and internal code of labor conduct at higher schools.

V. Control actions

5.1. The intermediate (half-annual) control is performed at training base in order to check how doctors (pharmacists)-interns learn the sections of curriculum, master the corresponding practical skills. 5.2. The final control is performed at completion of study cycle at training base before sending a doctor (pharmacist)-intern to the full time part of study at department of higher school. The composition and work schedule of commission is approved by the order from chief doctor of basic health care establishment, head of establishment (enterprise) within IU “Pharmacy”. The commission includes a person, responsible for internship at training base (chairman), direct supervisors of doctors (pharmacists)-interns, chief specialists from Ministry of Health Care of Crimean Autonomous Republic, health care departments at regional, Kyiv, Sevastopol city state administrations, chief state sanitary doctors in regions, city Kyiv and Sevastopol, head of regional IU “Pharmacy and of city Kyiv and Sevastopol, if possible, teachers of specialized departments at higher schools. 5.3. While studying at internship during 1.5-2 years the final half-annual (annual) control is performed at completion of the first cycle in full time part of study at department of higher school. 5.4. The final control includes the check on occupational practical training of doctor (pharmacist)-intern according to the plan and program, test control for level of knowledge and skills, interlocution. The practical part of final control for knowledge of doctors (pharmacists)-interns with certain profiles may start in advance, through participation of doctors (pharmacists)-interns in operations, delivery of babies, clinical, instrumental, sanitary epidemiological examinations, manufacturing processes, etc. in presence of teacher. 5.5. Doctors (pharmacists)-interns, who are not attested due to results from final control or failed to appear at it without valid reasons, shall pass through the final control during the periods, agreed with the dean’s office (division) of internship but not later than during two months since start of the new academic year. 5.6. The issues about possibility (expediency) for continuation of study of persons, who were not attested at final control, at internship, are solved by Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol at presentations of higher school. The unattested persons are subject to dismissal from internship by the order from Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol as those ones, who do not correspond to occupied post as a result from insufficient qualification and are sent to the Chief administration of state service and staff at Ministry of Health Care of Ukraine for solution of issue about their employment. 5.7. After expiration of training period at internship the doctors (pharmacists)-interns are subject to attestation after definition of knowledge and practical skills with awarding the title of doctor (pharmacist)-specialist in certain specialty according to the order by MHC of Ukraine, dd. 21.11.91 No 168 (v0168282-91) “On further improvement of doctors’ attestation”, dd. 4.02.92 No 24 “On further improvement in attestation of doctors and pharmacists”, dd. 9.06.93 No 130 (z0091-93) “On introduction of changes and amendments into the order from MHC of Ukraine, dd. 21.11.91 No 168 “On further improvement of doctors’ attestation”, dd. 1.06. 95 No 97 (z0319-95) “On introduction of changes and amendments into order from MHC of Ukraine”, dd. 21.11.91 No 168 “On further improvement of doctors’ attestation”. 5.8. The state attestation commissions are established every year to perform the attestation. Highly skilled experienced specialists are appointed to the post of chairman in state attestation commissions by the order from Ministry of Health Care of Ukraine. The composition of commission and timetable for performance of attestation is approved by the rector of higher school not later than during one month until start of attestation. The composition of state attestation commissions includes prorectors on academic and treatment work (prorectors of postgraduate education), dean of faculty (head at division) of internship (or his/her deputy), heads at departments, professors and assistant professors at specialized departments of higher school, chief specialists from Ministry of Health Care of Crimean Autonomous Republic, health care departments at regional, Kyiv, Sevastopol city state administrations, chief state sanitary doctors in regions, city Kyiv and Sevastopol, head of regional IU “Pharmacy and of city Kyiv and Sevastopol, professors and assistant professors of other higher schools and scientific research institutions, employees of trade unions, associations of doctors, etc. as members. 5.9. The following documents and materials are submitted to the commission to perform the attestation on definition of knowledge and practical skills with awarding the title of doctor (pharmacist)-specialist during one week until start of its meeting:

– letter of recommendation from administration of basic health care establishment (institution), where a doctor (pharmacist)-intern passed through training, record book, performance record book of doctor (pharmacist)-intern, 3×4 cm photo. 5.10. Doctors (pharmacists)-interns, who completely performed the curriculum and internship program, passed through final control for the previous time of study at internship in corresponding specialty, are admitted to attestation for definition of knowledge and practical skills with awarding the title of doctor (pharmacist)-specialist. The fulfillment of curriculum and program is certified by academic records, which are submitted by specialized and adjacent departments of higher school to the dean’s office of faculty (division) of internship not later than during one week until start of attestation. The admission of doctors (pharmacists)-interns to attestation for definition of knowledge and practical skills with awarding the title of doctor (pharmacist)-specialist is executed by the order from rector of higher school.

5.11. Organization and performance of attestation of doctors (pharmacists)-interns for definition of knowledge and practical skills: 5.11.1. The attestation of doctors (pharmacists)-interns includes the control of knowledge and skills by computer test programs, approved by Ministry of Health Care of Ukraine, assessment of knowledge and skills by state attestation commission, including possession of practical skills, interlocution or other form for final assessment of level in mastering the curriculum for interns. 5.11.2. The computer control of knowledge and skills is held at computer classroom of department (faculty) at higher school using test computer programs in corresponding specialties, approved by Ministry of Health Care of Ukraine, in the presence of members in state attestation commission. The assessment of training of doctors (pharmacists)-interns due to results from computer testing is performed due to binary system: “attested”, “unattested”. A doctor (pharmacist)-intern, who has not less than 75% of correct answers, is considered to be attested. At negative results from computer test control a doctor (pharmacist)-intern is considered to be such one that due to the level of training does not conform to requirements of job description for doctor (pharmacist) with appropriate specialty. In this case a doctor (pharmacist)-intern is not admitted to further stages of attestation and is considered to be unattested. 5.11.3. While performing the assessment of abilities and possession of practical skills the doctors-interns of treatment profile are subject to compulsory control of ability to conduct the examination of patient, to prescribe the treatment to a certain patient, to give the emergency aid (including the performance of cardiopulmonary resuscitation at trainer), to solve the issue about expertise of working ability. The control for possession of practical skills by doctors-interns of surgical profile is carried out in advance while making operations, medical maneuvers in the presence of members in state attestation commission and through analysis on professional activity of doctor-intern during internship. The control for possession of practical skills by doctors-interns with specialties of hygienic profile is carried out in advance when they perform the sanitary epidemiological supervision at facilities in the presence of members in state attestation commission through fulfillment of concrete situational tasks by doctors-interns and analysis on professional activity of doctor-intern during internship. At unsatisfactory assessment of skills and possession of practical skills a doctor (pharmacist)-intern is considered to be the one that did not mastered the practical skills, anticipated by curriculum, and did not conform to the requirements of job description for doctor (pharmacist)-intern due to level of training. In this case a doctor (pharmacist)-intern is not admitted to further stages of attestation and is considered to be unattested. 5.11.4. The interlocution or other form of final assessment of level in mastering the curriculum of internship is held with each doctor (pharmacist)-intern (due to test paper or ticketless methodology). The decision about level of training of doctor (pharmacist)-intern and awarding the title of doctor (pharmacist)-specialist in a certain occupation is taken due to results from interlocution taking into consideration the assessments of previous stages. 5.11.5. Due to results from attestation the state attestation commission may take the decision to award the title of doctor (pharmacist)-specialist in corresponding specialty, to refuse to award the title of doctor (pharmacist)-specialist. The decision is taken by open voting. At equal number of votes the vote by chairman in state attestation commission is the decisive one. The results from attestation are brought into knowledge of doctors (pharmacists)-interns at once after completion of the commission meeting. The results from attestation are executed by the protocol, which is signed by chairman and all members in state attestation commission and is approved by the order of higher school, where the commission was established, during ten days. 5.11.6. A person, who due to results from attestation for definition of knowledge and practical skills was awarded with the title of doctor (pharmacist)-specialist in a certain specialty, receives the certificate of established form from higher school (Annex No 4 to order from MHC of Ukraine. dd. 21.11.91 No 168 (v0168282-91) “On further improvement of doctors’ attestation), and a person, who was refused, receives the extract from commission meeting, certified by seal of higher school, during not later than three days since approval of protocol by meeting of state attestation commission. 5.11.7. The validity period for certificate of doctor (pharmacist)-specialist is 5 years until the regular attestation for qualification category after passing through corresponding pre-attestation cycle of study at department of higher school. Doctors (pharmacists), who do not wish to be attested for qualification category in 5 years after receiving the certificate of doctor (pharmacist)-specialist, shall be attested to confirm the title of doctor (pharmacist)-specialist after passing through the study at cycle of qualification improvement at department of higher school on corresponding occupation. At failure to confirm the title of doctor (pharmacist)-specialist or at failure to pass through attestation for qualification category in 5 years after receiving the certificate of doctor (pharmacist)-specialist, such person is transferred to the post of doctor (pharmacist)-trainee. The repeated attestation to confirm the title of doctor (pharmacist)-specialist may be held not earlier than in a year after transfer to the post of doctor (pharmacist)-trainee. 5.11.8. The decision by attestation commission may be appealed at Ministry of Health Care of Ukraine during two weeks since issue of extract from protocol by meeting of state attestation commission. Due to decision by Ministry of Health Care of Ukraine a doctor (pharmacist)-intern may be re-attested at attestation commission of other higher school. 5.11.9. The meeting of state attestation commission in certain cases with agreement by Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol may have the visiting session and be held at training bases. 5.11.10. During two weeks the chairman of state attestation commission submits the report about work of commission to the Ministry of Health Care of Ukraine through the dean’s office (division) of internship at higher school. 5.11.11. The possibility for repeated attestation for definition of knowledge and practical skills with awarding the title of doctor (pharmacist)-specialist may e granted to a doctor (pharmacist)-intern once (on cost accounting basis) during three years but not earlier than in a year after refusal in awarding the title of doctor (pharmacist)-specialist according to s. 3.12.3.5. Provisions about organization of academic process at higher schools, approved by order from Ministry of Health Care of Ukraine, dd. 2.06.93 No 161 (z0173-93).

VI. Transfer, dismissal and resume at internship

6.1. The transfer of doctor (pharmacist)-intern from one to another higher school is carried out with permission from Chief administration of educational establishments at Ministry of Health Care of Ukraine and with agreement by rectors of higher schools and Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol, where he/she was sent or transferred to study or work. A doctor (pharmacist)-intern submits the record book, certified by the seal of higher school, from which he/she is transferred, a diary, signed by head of internship and certified by higher school and head of health care establishment (institution) – training base, individual curriculum with the record about progress in its fulfillment, to the higher school at the place of transfer. 6.2. The transfer of doctor (pharmacist)-intern from one to another training base is carried out with permission from Chief administration of educational establishments at Ministry of Health Care of Ukraine and with agreement by rectors of higher schools and Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol and higher school, on which base the study of doctor (pharmacist)-intern at department is held. A doctor (pharmacist)-intern submits the extract from order by Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol, from training base, from which he/she is transferred, a diary of doctor (pharmacist)-intern and individual curriculum with the record about progress in its fulfillment. The orders from Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol mention at the expense of which health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol the internship will be financed further. 6.3. The expulsion from internship is carried out for failure to fulfill the curriculum and program, for infringement of internal code of labor conduct, for missing classes (including due to valid reasons if a doctor (pharmacist)-intern missed more than one third of classes at department), for working disability if due to conclusion by medical consulting commission (MCC) or medical social expert commission (MSEC) the disability of doctor (pharmacist)-intern to work as a doctor (pharmacist) was determined. The expulsion of a doctor (pharmacist)-intern from cycle of full time part of study at department of higher school is carried out by the order from rector of higher school, which copy is sent to Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol. The expulsion from internship is carried out by the order from Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol at presentation of higher school or administration of training base. 6.4. The resume at internship is allowed for persons, who were dismissed from internship due to valid reasons (health conditions) and is carried out pursuant to the written application, addressed to the minister of health care of Crimean Autonomous Republic, head if health care department at regional, Kyiv, Sevastopol city state administration, chief state sanitary doctor in region, city Kyiv and Sevastopol, head of regional IU “Pharmacy” and of city Kyiv and Sevastopol with agreement from higher school providing that a doctor (pharmacist), who is resumed at internship, is enrolled to the post of doctor (pharmacist)-intern. The resume of persons, who were dismissed from internship for infringement of internal code of labor conduct and failure to perform the curriculum and program (without valid reasons) is made pursuant to the contracts that are concluded between Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol, higher school and legal entities and natural persons, who will finance the study at internship. 6.5. The change of specialty at internship may be performed due to disability according to conclusion by MCC or MSEC it was defined that a doctor (pharmacist0-intern is disable to pass through internship in this occupation, due to business needs that require the consent from doctor (pharmacist)-intern. The change of specialty at internship is carried out by the order from Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol with agreement from higher school, from which a doctor (pharmacist)-intern graduated. The change of specialty “medical preventive care” for specialty “general medicine” and “pediatrics” and vice versa may be carried out only with agreement from Ministry of Health Care of Ukraine pursuant to petition from Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol.

VII. Financial provision of internship

7.1. The journey of young specialists, who have the job placements, to the place of specialization (internship) and the monthly vacation until start of training at internship is paid by the customer, where they are sent for work. 7.2. The journey of doctors (pharmacists)-specialists, who graduated from internship, to the place of work, is paid by establishments (institutions), where they are sent after graduation from internship for constant work. 7.3. The salary to doctors (pharmacists)-interns during the whole period of study is paid by establishments (institutions), to which they are enrolled as doctors (pharmacists)-interns, by establishment (institution), with which the labor agreement is concluded, in the amount, established by existing legislation. 7.4. During internship the doctors (pharmacists)-interns are provided with the living area (of hostel type) of those establishments, on which base their training is held, with the payment according to existing rates. If the hostel is absent a health care establishment (institution) is entitled to lease the living area for doctors (pharmacists)-interns at other organizations or rent it at private persons with repayment of actual costs at the expense of doctors (pharmacists)-interns in the amount of payment, which is established by existing legislation, and the rest of the sum – at the expense of this establishment (institution). 7.5. The expenses on business trip of doctors (pharmacists)-interns to study at higher school are paid by basic training establishment due to the same procedure as while sending the specialists for improvement of qualification. 7.6. The number of posts of teachers at higher schools is defined on the basis of one teacher per 5 average yearly doctors (pharmacists)-interns. 7.7. The payment for supervision for training of doctors (pharmacists)-interns is made to their supervisors for the period of work at training bases according to existing legislation. 7.8. The training of doctors (pharmacists)-specialists at internship for health care establishments of other ministries, departments, organizations, enterprises, other institutions, regardless of pattern of ownership, is financed at the expense of their funds pursuant to contractual terms. 7.9. It is allowed for graduates from higher medical and pharmaceutical schools of III-IV accreditation levels in medical faculties of universities, who received the right for self-employment, to pass through training at internship due to a separate agreement between a graduate and Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol, higher school. 7.10. Doctors (pharmacists) pass through training at internship pursuant to the terms of contracts, which are concluded between Ministry of Health Care of Crimean Autonomous Republic, health care department of regional, Kyiv and Sevastopol city state administration, regional sanitary epidemiological station and of city Kyiv and Sevastopol, regional IU “Pharmacy” and of city Kyiv and Sevastopol, higher school and legal entities and natural persons, who will finance the study at internship, if these doctors (pharmacists) graduated from non-state higher medical (pharmaceutical) schools of III-IV accreditation levels in medical faculties of universities, or graduated from state higher medical (pharmaceutical) schools of III-IV accreditation levels in medical faculties of universities, studying at them for the funds of legal entities and natural persons, or employed to non-state health care establishments (non-state pharmaceutical establishments, enterprises), or have the certificate of doctor (pharmacist)-specialist and wish to acquire other specialty if there is no need in such specialist at health care establishment (institution).
